This document is to declare under the penalty of perjury that you did the course by yourself meaning you did not ask anyone to do it for you.

DO NOT PANIC IF SOMEONE WAS HELPING YOU STUDYING. 

That is perfectly normal to get help from a parent or sibling as long as you were the one studying.

The important thing is for your subconscious mind knowing right away what to do in case of an emergency so it can take command even before you have time to think.
